Please arrive 15 minutes early to your first appointment to fill out paper work. Make sure you bring your insurance card and a photo ID; we will need to make a copy for your chart.

Please park in the back/South (behind the building) U-shape parking lot and go in the doors with the 800 above the door, we are the first suite on the left.

Note: After 5:00pm or on weekends-only the back/south entrance is unlocked. If a door is locked, you are at the wrong entrance.
